I-485 receipt notice (I-797)

The original notice mailed after filing your application.
Proves your application was properly filed and accepted by USCIS.
Government-issued photo ID
Driver's license or state ID card. Must be current.
A second form of ID helps USCIS confirm your identity at interview.
2 passport-style photos
White background, taken within 30 days of interview.
USCIS may retake your photo, but bring these as backup.
Birth certificate + translation
Certified translation required if not in English.
Primary proof of your date and place of birth.
Marriage certificate (if applicable)
Official copy, with certified translation if needed.
Required for spousal petitions to prove the qualifying relationship.
I-693 medical exam (sealed)

Original civil surgeon report in sealed envelope.
Proves you meet health requirements. Do not open the sealed envelope.
